The frail elderly person at the centre of cohesive care (WC2009-053)

Currently, three significant problems can be identified in the Dutch health care system for older adults:

  • older adults experience a lack of autonomy in their care process; possibilities for self-management of frail elderly persons and their informal carer(s) are insufficiently recognized
  • care is fragmented and coordination between professionals is lacking
  • care providers and elderly persons are reactive, health risks of frail elderly persons are detected late and are inadequately followed up

This project aims to target these problems by introducing the Geriatric Care Model (Muntinga et al., 2012), based on the Chronic Care Model.
